Important Update

 

Office Closure Mailing Address Change

Due to funding restrictions and downsizing, we have been forced to make the dessision to not renew our lease at our dedicated office in downtown Clarksville. Because of this change we will no loinger be taking walk-in appoitments and our mailiong address has changed.

The new address is 2918 Heatherfield Drive, Woodlawn, TN 37191.

Client Communication Policy

Clients will NOT contact staff of Meae Wellness or Syndemic Response Alliance on staff’s personal devices. In the future, any communication received on personal devices by staff will be instructed to contact the Syndemic Response Alliance Hotline at (833) 943-4448 for assistance. This change is due to staffing changes, client security, and privacy, and will be a zero tolerance policy.

ProTransport Policy & Procedure Changes

If not done already, staff will be contacting clients to provide updated “Notice of ProTransport Policy & Procedure for Syndemic Response Alliance (SRA) Prevention and Intervention.” This will replace the previous associated document. It has changes that include updated Federal Poverty Levels and updated wordage reflecting program changes.

Mask Policy

In a continued commitment to reducing the effect of COVID-19, Meae Wellness will continue to require masks to be worn by clients, staff, and visitors in our facilities, vehicles, or when in close proximity. This requirement is IN ADDITION to encouraging vaccination. This means masks will be required even if an individual has received their COVID-19 vaccination.
